La cadera está metida hacia adentro, muy por debajo del músculo. No se hincha a la vista y no se puede tocar con la mano. Por eso se descubre tarde.The hip sits deep inside, well under the muscle. It doesn't swell visibly and you can't feel it with your hand. That is why it gets discovered late.
Y por eso confunde tanto. El dolor de cadera baja por el muslo y a veces llega hasta la rodilla. - Artrosis primaria. Desgaste con los años, sin una causa única identificable.Primary osteoarthritis. Wear over the years, with no single identifiable cause.
- Pinzamiento femoroacetabular. Una forma de la cadera que roza al flexionar; en el paciente joven es una causa frecuente y tratable.Femoroacetabular impingement. A hip shape that rubs when you bend; in younger patients it is a frequent and treatable cause.
- Displasia. Una copa poco cubierta que sobrecarga el borde del cartílago durante años.Dysplasia. A shallow socket that overloads the edge of the cartilage for years.

El síntoma más característico. Dolor inguinal que aparece al caminar y cede al descansar. Muchos pacientes se agarran la ingle o hacen una C con la mano sobre la cadera al describirlo.The most characteristic symptom. Groin pain that shows up walking and eases with rest. Many patients grip the groin or cup a C-shape over the hip as they describe it.
"No me puedo poner las medias""I can't put my socks on"
Pérdida de rotación y flexión. Cortarse las uñas, amarrarse los zapatos o cruzar la pierna se convierten en maniobras. Suele ser la señal más temprana.Loss of rotation and flexion. Cutting toenails, tying shoes or crossing your legs turn into maneuvers. This is usually the earliest sign.

Ojo con esta confusión, que es diaria: el dolor en el costado de la cadera —el que molesta al dormir de ese lado y al presionar el hueso lateral— casi nunca es artrosis. Suele ser tendinopatía glútea o bursitis, y se trata sin cirugía, fortaleciendo la cadera. Artrosis duele en la ingle; el costado es otra cosa. Distinguirlo evita años de tratamiento en la dirección equivocada.Watch out for this confusion — it happens daily: pain on the side of the hip — the kind that bothers you sleeping on that side and hurts when you press the bone — is almost never arthritis. It is usually gluteal tendinopathy or bursitis, and it is treated without surgery, by strengthening the hip. Arthritis hurts in the groin; the side is something else. Telling them apart saves years of treatment in the wrong direction.
