Una infiltración es la aplicación de un medicamento directamente dentro de la articulación o junto al tejido lesionado. Existen varios tipos y no todos hacen lo mismo. Cuál tiene sentido en tu caso —o si ninguno lo tiene— depende del diagnóstico, no del catálogo. Eso se define en la valoración.An injection is the delivery of a medication directly into the joint or alongside the injured tissue. Several types exist and they don't all do the same thing. Which makes sense in your case — or whether none of them does — depends on the diagnosis, not the catalog. That is determined at the consultation.
La gente las conoce de muchas formas: «la inyección de gel», «las de gallo», «la del desgaste», «las viscosas», «el plasma». Debajo de esos nombres hay cosas distintas. Vamos una por una — pero primero, por qué el orden importa.People know them by many names: "the gel injection", "rooster comb shots", "the wear-and-tear shot", "the viscous ones", "the plasma". Mucha gente llega al consultorio con dos miedos al tiempo: que le digan que necesita cirugía, y que le vendan algo para no operarse. Hay pacientes que se benefician de una infiltración, otros que necesitan fortalecer antes de cualquier aguja, y otros a quienes lo más honesto que puedo decirles es que el quirófano ya es la mejor opción para su caso.My job is not to push you in any direction. It's to tell you where you stand. Some patients benefit from an injection, others need to strengthen first before any needle, and for others the most honest thing I can tell them is that surgery is already the best option for their case.

Una rodilla que duele no siempre duele por lo mismo. Puede ser artrosis, una lesión meniscal, una tendinopatía, dolor referido de la cadera o de la columna. Cada diagnóstico responde distinto — y algunos no responden a ninguna infiltración.A knee that hurts doesn't always hurt for the same reason. It could be osteoarthritis, a meniscal injury, a tendinopathy, or referred pain from the hip or spine. Each diagnosis responds differently — and some don't respond to any injection.
Por eso la consulta empieza con examen físico, revisión de tus imágenes, ecografía en el consultorio cuando aporta, y medición objetiva de fuerza con dinamometría. Solo con un diagnóstico sobre la mesa tiene sentido hablar de qué se puede aplicar.That's why the consultation begins with a physical exam, review of your imaging, in-office ultrasound when it adds value, and objective strength measurement with dynamometry. Only with a diagnosis in hand does it make sense to talk about what can be applied.

Cómo funciona: reduce la inflamación local de forma rápida; su efecto va de semanas a algunos meses. Es un recurso puntual dentro de un plan más amplio, no una solución permanente ni algo que se repita indefinidamente.How it works: reduces local inflammation quickly; its effect lasts from weeks to several months. It is a targeted resource within a broader plan — not a permanent solution, and not something repeated indefinitely.

Qué es: una sustancia viscosa parecida a la que produce naturalmente la articulación sana.What it is: a viscous substance similar to what a healthy joint naturally produces.
Cómo funciona: busca mejorar la lubricación y la amortiguación dentro de la articulación. No reconstruye cartílago perdido.How it works: seeks to improve lubrication and cushioning inside the joint. It does not reconstruct lost cartilage.
Cuánto dura: en el paciente correcto puede dar un alivio sostenido — en mi experiencia, algunos pacientes se mantienen bien más de un año. Esa pregunta no tiene una sola respuesta, y desconfía de quien te la dé sin haberte examinado.That question has no single answer — and be wary of anyone who gives you one without having examined you.
La evidencia científica de cada terapia no es una nota fija: cambia según el diagnóstico, la articulación, el grado de desgaste y el momento en que estás. Una misma sustancia puede tener respaldo razonable en un escenario y ninguno en otro. Por eso no vas a encontrar aquí una lista de «la número uno»: sería un titular cómodo y una media verdad.The scientific evidence for each therapy is not a fixed score: it changes depending on the diagnosis, the joint, the degree of deterioration, and where you are right now. The same substance may have reasonable support in one scenario and none in another. That's why you won't find a "number-one" list here: it would be a comfortable headline and a half-truth.

Cuándo no se infiltra.When not to inject.
Decir que no también es parte del trabajo. Estas son situaciones en las que una infiltración no es la respuesta:Saying no is also part of the job. These are situations in which an injection is not the answer:
Con dolor permanente, limitación marcada y desgaste avanzado, una infiltración suele aplazar unas semanas una decisión que ya está tomada.With constant pain, significant limitation, and advanced degeneration, an injection usually postpones by a few weeks a decision that has already been made.
Ninguna de estas terapias reconstruye cartílago perdido. Si esa es la expectativa, se corrige antes de aplicar nada, no después.None of these therapies rebuild lost cartilage. If that is the expectation, it is corrected before anything is applied — not after.
En la piel del sitio de punción o dentro de la articulación. Es contraindicación, no una precaución negociable.In the skin at the injection site or inside the joint. It is a contraindication, not a negotiable precaution.
Un dolor de rodilla que en realidad nace en la cadera o en la columna no mejora infiltrando la rodilla. Por eso el diagnóstico va primero.Knee pain that actually originates in the hip or spine does not improve by injecting the knee. That is why diagnosis comes first.

Buena parte de las infiltraciones se aplican por referencias anatómicas: calculando desde afuera dónde está la articulación. La ecografía permite ver la aguja entrando y confirmar que el medicamento queda donde tiene que quedar.Many injections are applied using anatomical landmarks: estimating from the outside where the joint is. Ultrasound allows us to see the needle entering and confirm that the medication ends up where it needs to be.
La diferencia importa: si el medicamento no llega al sitio correcto, ninguna discusión sobre cuál es mejor aplica a tu caso.The difference matters: if the medication doesn't reach the right place, no discussion about which one is best applies to your case.
Qué muestra la literatura: las revisiones sistemáticas coinciden en que la guía ecográfica mejora la precisión de colocación frente a la técnica por referencias anatómicas, con mejores desenlaces a corto plazo.What the literature shows: systematic reviews agree that ultrasound guidance improves placement accuracy compared to the landmark-based technique, with better short-term outcomes.

En algunos pacientes puede aplazar la cirugía o hacerla innecesaria; en otros no. Depende del diagnóstico y del grado de desgaste, y la respuesta honesta solo existe después de la valoración.In some patients it may delay surgery or make it unnecessary; in others it won't. It depends on the diagnosis and the degree of deterioration — and the honest answer only exists after the evaluation.
¿Cuántas veces me puedo infiltrar?How many times can I get an injection?
No hay un número universal. Depende de la terapia, del diagnóstico y de cómo respondiste a la anterior. En el caso del corticoide, la aplicación repetida y frecuente se evita.There is no universal number. It depends on the therapy, the diagnosis, and how you responded to the previous one. In the case of corticosteroids, repeated and frequent application is avoided.
¿El PRP es mejor porque es «de mi propia sangre»?Is PRP better because it comes from "my own blood"?
Que venga de tu sangre reduce ciertos riesgos, pero eso por sí solo no lo hace más eficaz. Si es una opción razonable para tu caso lo conversamos con la evidencia que existe para tu diagnóstico.Coming from your blood reduces certain risks, but that alone does not make it more effective. Whether it is a reasonable option for your case is something we discuss using the evidence that exists for your diagnosis.
¿Duele?Does it hurt?
Se usa anestesia local y la mayoría de pacientes lo tolera bien. Puede quedar molestia o presión algunas horas.Local anesthesia is used and most patients tolerate it well. There may be discomfort or pressure for a few hours.

Depende del diagnóstico y del plan, así que se cotiza después de la valoración. Una infiltración sola rara vez resuelve. Lo que se hace después, sí.An injection alone rarely resolves the issue. What comes after, does.
Ninguna terapia inyectable es un tratamiento completo por sí misma. Lo que hace, en el paciente adecuado, es abrir una ventana: bajar el dolor lo suficiente para que puedas hacer el trabajo que de verdad cambia tu articulación — recuperar fuerza y corregir el movimiento que te está lastimando.No injectable therapy is a complete treatment by itself. What it does, in the right patient, is open a window: reduce pain enough so you can do the work that truly changes your joint — recovering strength and correcting the movement that is causing the damage.
Si esa ventana se desaprovecha, el dolor vuelve y la conclusión equivocada es "la inyección no sirvió". Por eso toda infiltración se indica junto con un plan de rehabilitación, que trabajamos con Movid, el centro de medicina deportiva donde también paso consulta. Y si prefieres otro centro, entrego el protocolo por escrito. Tu plan no se parte en dos.If that window is wasted, the pain returns and the wrong conclusion is "the injection didn't work". That's why every injection is indicated alongside a rehabilitation plan, which we work on with Movid, the sports medicine center where I also see patients.
