Si te operaron y la cadera sigue doliendo, no estás exagerando y no eres "el paciente difícil". Que una prótesis duela siempre tiene una explicación. A veces la explicación es sencilla y la solución no incluye otra cirugía.
Lo primero es encontrar la causa. Eso lleva una consulta con examen, unas radiografías y casi siempre unos exámenes de sangre. Con eso en la mano ya se puede decir qué está pasando y qué se hace.
Y una cosa importante: que una prótesis falle no significa que alguien lo hizo mal. Las prótesis se desgastan, el hueso cambia, uno se cae. Aquí no venimos a buscar culpables. Venimos a resolver tu cadera.
If you were operated on and the hip still hurts, you are not exaggerating and you are not "the difficult patient". A replacement that hurts always has an explanation. Sometimes the explanation is simple and the solution does not involve another operation.
The first step is finding the cause. That takes a visit with an exam, some X-rays and, almost always, blood tests. With that in hand we can already say what is happening and what gets done.
And something important: a replacement failing does not mean someone did something wrong. Implants wear, bone changes, people fall. We are not here to look for someone to blame. We are here to solve your hip.

Hay molestias normales después de una prótesis de cadera, sobre todo el primer año:There are normal complaints after a hip replacement, especially in the first year:

Eso suele mejorar solo. Lo que sí hay que estudiar es el dolor que aumenta con el tiempo en vez de bajar, el que aparece de nuevo después de meses buenos, o el que viene con fiebre o con la herida caliente.Those usually settle on their own. What does need investigating is pain that grows instead of easing, pain that returns after months of feeling well, or pain that comes with fever or a hot wound.

La prótesis se aflojóThe implant has loosened
Con los años, la unión entre el implante y el hueso puede perder agarre. El dolor aparece al apoyar el peso y al empezar a caminar, y se calma con el reposo. Se ve en las radiografías comparadas con las de antes — por eso vale oro traer las viejas.Over the years, the bond between implant and bone can lose its grip. The pain shows up when you put weight on it and when you start walking, and it eases with rest. It shows on X-rays compared with older ones — which is why bringing the old films is worth gold.
Hay una infecciónThere is an infection
Es la causa que nunca se puede pasar por alto, porque cambia todo el tratamiento. Puede aparecer temprano, con fiebre y herida caliente, o mucho después, como un dolor sordo que no se quita nunca. Se descarta con exámenes de sangre y, si hace falta, sacando una muestra de líquido de la articulación. Se estudia siempre, aunque parezca poco probable.This is the cause that can never be missed, because it changes the whole treatment. It can appear early, with fever and a hot wound, or much later, as a dull ache that never goes away. It is ruled out with blood tests and, if needed, by taking a sample of fluid from the joint. It is always investigated, even when it seems unlikely.

If it has happened more than once, we need to find out why — it usually has to do with how the implant was oriented or with weak muscles around it. And it has a solution.

That wear can irritate the surrounding bone and loosen the implant. It is one reason a periodic check-up is worth it even when you feel well: it is caught before it hurts.

Suele ser después de una caída, aunque a veces basta con un golpe pequeño si el hueso está débil. Es una cirugía exigente, porque hay que arreglar el hueso teniendo un implante adentro. Es justamente el campo en el que me formé con un máster dedicado solo a esto.It is usually after a fall, though sometimes a small knock is enough if the bone is weak. It is a demanding operation, because the bone has to be fixed with an implant inside it. That is precisely the field I trained in with a dedicated master's program.

Si la copa o el vástago quedaron en un ángulo que no corresponde, la cadera se sale con facilidad, roza donde no debe o deja una pierna más larga que la otra. Se mide con una tomografía y se corrige.If the cup or the stem ended up at the wrong angle, the hip dislocates easily, rubs where it shouldn't, or leaves one leg longer than the other. It is measured with a CT scan and corrected.

The implant gets investigated and comes out perfect, and the pain continues — because it never came from there. Finding that avoids an operation that wasn't needed.
Cómo se estudiaHow it is investigated

Cuándo empezó a doler, si alguna vez estuviste bien después de operarte, qué lo empeora. Si nunca dejó de doler desde el día uno, se piensa en unas causas; si dolió después de meses buenos, en otras.When it started hurting, whether you were ever well after surgery, what makes it worse. If it never stopped hurting from day one, we think of some causes; if it hurt after good months, of others.

Para descartar infección. Se piden casi siempre, aunque todo apunte a otra cosa.To rule out infection. They are ordered almost always, even when everything points elsewhere.
La tomografía mide cómo quedó puesto el implante. La muestra de líquido confirma o descarta la infección. No siempre se necesitan.The CT measures how the implant was placed. The fluid sample confirms or rules out infection. They are not always necessary.
¿Y si hay que volver a operar?And if it has to be operated again?
Se llama cirugía de revisión: cambiar todo el implante o solo la parte que está fallando. Es más larga y más exigente que la primera, porque hay que sacar lo que está puesto, ver cómo quedó el hueso y reconstruir lo que falte. Por eso no la hace cualquiera y por eso se planea con tiempo.
Tres cosas que conviene saber desde ahora:
- ✚No siempre se cambia todo. Si una sola pieza es la que falla, muchas veces se cambia solo esa.
- ✚Si hay infección, el camino es distinto. A veces se hace en dos tiempos: primero se limpia y se trata la infección, después se pone la prótesis nueva. Es más largo, y es la forma de que quede bien.
- ✚La recuperación es más lenta que la primera vez. Se avanza igual, pero con más calma. Vale la pena saberlo antes y no después.
Y también hay que decirlo: no todo caso necesita otra cirugía. Cuando el dolor no viene de la prótesis, operar no lo quita. Encontrar eso a tiempo es tan valioso como una buena cirugía.
It is called revision surgery: replacing the whole implant, or only the part that is failing. It is longer and more demanding than the first one, because what is in place has to come out, the state of the bone has to be assessed, and whatever is missing has to be rebuilt. That is why not everyone does it and why it is planned with time.
Three things worth knowing from now:
- ✚Not everything is always replaced. If a single part is failing, often only that part is changed.
- ✚If there is infection, the path is different. Sometimes it is done in two stages: first the infection is cleaned out and treated, then the new implant goes in. It is longer, and it is how it ends up right.
- ✚Recovery is slower than the first time. The progress is the same, taken more calmly. It is worth knowing beforehand rather than after.
And it has to be said: not every case needs another operation. When the pain doesn't come from the implant, operating won't remove it. Finding that in time is as valuable as a good operation.

Beyond hip and knee surgery, I did specific training in fractures around implants and in pelvic and acetabular surgery. Those are the two hardest situations in this specialty, and the ones fewest surgeons in the country handle.
That matters even if you never need it: it means the surgeon operating on you the first time knows what to do if something gets complicated, instead of having to send you to another city.
